AkA Statement

I have a signing this afternoon with an aka statement.
I am in CA. and this reads:
At the top:
State of Washington
County
At the bottom:
Notary signature
Exp. date.
I know I cannot decide on my own to use an ack. or jurat and of course cannot get hold of anyone on the "emergency contact" line. Any suggestions would be appreciated. Thank you.

Most of the ones I see are jurats, they are swearing they are also known as .I would attach a loose jurat or if room use jurat stamp and line thru the one on page. Jmo of course.

ARe you sure there isn't any verbiage anywhere within the document to give you a clue as to what kind of a notarization to do? I have also seen these with acknowledgements. And yes, don't forget to change the state.
