Can someone helpme with this document? It reads

I am not sure what goes after the word ,by... Is it the borrowers name or my name? Please help

You are joking, right? This is the most basic of notary knowledge there is. You need to know what an acknowledgment is and how to be state compliant with the proper form and syntax. Read your notary manual on how to prepare a CA compliant acknowledgment (assuming you are in CA).

Well, considering that the ack is not CA compliant, I can see where it might be confusing. If I'm correct, CA requires that the new acknoledgment be used. This ack can be found on page 11 of the 2006 manual.
