Citibank for NASCO. Form question

I have a Citibank HELOC for NASCO in which I see a "Form of Identification Affidavit/Mortgagors". Are we to fill this out AND notarize it?

Is there notarial wording on the document? If not then you do not notarize. If so you will have the borrower fill out the information then notarize. Just remember that we are not to prepare the documents that have signatures to be notarized.

That's why I was wondering if we had to notarize it. It asks for me to fill in the ID information I receive on the borrower's then notarize it at the bottom.

Yes, this is to be notarized. Borrowers sign and write in SS#.

So the borrower enters their SS# then signs it and I fill in the rest and notarize it?

You fill in all the info from DL, the main borrower signs at the top and puts in SS# and the co-borrower signs at the bottom and puts in SS#.

Also, don't forget to print out 2 sets of the DOT. Also, make sure you read Page 3 of the Closing Instructions to see if you need to collect the borrowers' Declaration Page of their Homeowners Insurance.

I checked page 3 under "Notary Conditions/Instructions" and I see nothing re: the Declaration page. It mentions instructions for returning the Funding Transmittal Package and what all the borrower's must sign.

Look at the top of the page where it says "Add'l Borrower Conditions to Close". It may or may not be there. If not, then you don't have to get a copy of this doc.

One more thing, always collect a copy of their DL. It is required.

I was told a long time ago that we didn't have to do both and it's never been a problem.

I have had one or two occasions where I did likewise and was never called on it. However, if they ask for it, I do everything I can to produce it.

Both get a copy of the driver's license and fill out the ID form. You only need to do one or the other, preferrably get a copy of the license.

Cali may be right but this is what I have always done. I started doing these 3 years ago and maybe back then it was requested so I have just always done it and have never been told otherwise by NASCO. I always request the Declarations Page too when I call the borrowers to confirm the appt. because you don't know until you actually receive the docs. if you have to get a copy of this also. If you're unsure what to do, call NASCO.
