Direct Debit Enrollment Form

Did not want to ask, but could not find any info regarding the Direct Debit Enrollment Form in loan package that I have for signing this evening. Need to know if bwr needs to fill this out in my present to return with loan package.

Only if they want theri mtg. pymts. debited from their bank account.
If yes, they want that, then have them fill it out and return with the pkg.
Most of the brw's I have seen decline that at the time, I remind them they have another copy in their paperwork should they change their minds.
If the brw. doesn't want it, I alwasy have them write "declined" on the top of the form with their initials, so the lender/TC knows the brw was predented with the form and decided not to take the auto debit, instead of me deciding for them/not presenting the form.

One of my first bad experiences as an agent, was when the brw declined, but I had them write nothing on the form, just attached a sticky note with my writing he declined.
The L/O tried to accuse me of not showing the brw the form.
