I, _________, a NP in and for said county and st., do hereby certify that ________ personally known to me to be the same person(s) whose name(s)_____________subscribed to the foregoing instrument, appeared b4 me this day in person & acknowledged that he _______________signed and delivered the said instrument as _________ free voluntry act, for the uses & purposes therein set forth.
I haven't ever seen one like this before, and there were 2 borrowers with long names and yet the lines were so short! I wasn't sure what exactly to put on the last 2 lines. I'm only used to completing the first 2 lines listed. This one was WEIRD to me.

If names are too long to fit on first blank just caret them above the line
You are fortunate that I saw this first while waiting for docs to print Some would be way less patient and tell you to go back to school, etc. You might want to check a Signing agent reference, or just read through posts here. This subject has been covered Good Luck

Now Bob, what would we possibly say in a situation like this? That he should already know what is and isn't acceptable wording in his state? That it is completely unacceptable to leave blank spaces in a notarization? That he just failed Notary 101 by not knowing how to do the most common notary task?
Yeah, that sounds about right to me.

Those are wording of a general ackowledgment without the preprinted text in the blank spaces.
I,...(your name) certify that...(signer's name) whose name(s)....(is/are) acknowledged that...(he/she/they) instrument as...(his/her/their)

I can't help it if I've NEVER SEEN that wording. I've done 76 signings, and I've NEVER seen it worded THAT PARTICULAR way. I was just telling the truth!
