Medical release letter

Hi all,
My Huband and I are going on a cruise and the kids will be staying with my in-laws.
Want to leave them with a letter giving them permission to seek medical attention if
needed. Anyone have such a form?

Under Florida's HIPAA rules, you may need to execute a DPOAHC (Durable Power of Attorney for Health Care) or treatment may not be provided. Check with your personal physician, clinic and hospital to see if they require such a document. They may also have the 'blank' forms for you to complete.
