He paid me right away too, except for shorting me for the second trip fee for a refi aff that wasn't included in the original package. And now the fax line is disconnected and he's not responding to inquiries via phone or email.

Try to call him at EOM, and block your calling number so he will answer. I did it a few months
ago and it worked, he paid me in a week. Never will work for him.
