First thing you do is stop using all CAPS, that is considered yelling and it's hard to read. The next thing that you do is to tell that person that you are not an attorney and can not draft something like that. There are all kinds of ways to transfer title, the simplest being a grant deed, but is that what this person really wants to do? You are not qualified to answer that question for them, they need to contact an attorney.

As I think you already know, you cannot do the docs to make that happen. You can only notarize a document. It is not up to you to decide whether or not the document will do the job intended. You just notarize a signature.

If this is in connection with a loan signing, you could simply pass along to whoever hired you that borrower wants to know if it's possible to make a change to title while they are signing papers. I did that once and the title company sent over a QCD. But I certainly didn't draft it or tell the borrowers what to do.
The best advice I ever got about being a notary is that you notarize a SIGNATURE, not a document, so your part is the same whether the document is a 500-page contract or a statement that says, "My favorite flavor of ice cream is butter pecan."
My speech to the borrowers: "I'm an independent notary public. I don't work for your lender or the title company and I didn't prepare these documents. My job is to make sure you're who you say you are, that you sign in the right place, and that nobody's holding a gun to your head making you sign. If you have any questions about your loan, we'll need to get someone else on the phone to answer those."
