An acknowledgement is when the signer says out loud "Yeah, I signed that document of my own free will" or words to that effect. A certificate of acknowledgement is where the notary writes that the signer acknowledged his/her signature. If someone is asking for a certificate of acknowledgement, it should either be written on the document that was acknowledged, or attached to it. If a lender wants a blank one with your signature and/or seal, refuse.

A certificate of acknowledgement is more or less the same as an individual acknowledgement FORM, but of course corporate, partnership, and all other varieties of acknowledgement forms are also certificates of acknowledgement.

I have never heard of a certificate of acknowledgment

I have heard of certificates, and I have heard of acknowledgments. But never a "certificate of acknowledgment".

I think this means an acknowledgment, but I'm not in your state.
