Help Needed Please

I have come across unusual language in the notary block. Please advise how you would fill in the blanks. This is for a single individual signer. Many thanks.

On (date), before me, a Notary Public in and for said County and State, personally appeared (signer) , the individual who executed the foregoing instrument and acknowledged that __________ did examine and read the same and did sign the foregoing instrument, and that the same is ________ free act and deed.

What CK said, but don't forget you must also include the type of identification used to identify the signer.
