updated Customer ID Verification form

Has anyone come across this updated one - it is in the DocMagic packages - the form on long paper with the boxes in the middle? It was updated 9/11/06. Under the section where you are to explain 'discrepancies' with the id there is a line that says borrower's initials - and there are 6 lines. Of course there is just one borrower per sheet so I've got no idea what those additional lines are for although I digress. Do you think this is for the borrower to initial every time or just if there is a discrepancy. I've been having them do it each time and putting a n/a on the discrepancy lines - previously I never put anything on that line. I sometimes wonder who creates these 'new and improved' documents.
