In California providing a legal form to a signatory other than an Acknowledgement or Jurat is considered the Unlicensed Practice of Law (UPL). If you do this you could put your commission in jeopardy. DON'T DO IT!!!

Not to be too pickey, but when I am down at the courthouse looking for forms, they are forbidden to tell you which one to choose, becuase that is giving legal advice. But they can give you any one that you request. So I get them all and sort them out when I get home.

I think if a notary is providing a form for someone else, that's trouble. But if you need it for yourself, that's your own problem. But if you have your own trust, you got it from an attorney, and he should be able to provide you with the certification of trust.

I think you may be asking for trouble here. As a California notary you are not authorized to issue certifications of any document other than an entry from your official journal.
And as others have stated, you are not to advise any signor as to which form to be used in any notarial act.
