Have them fax the page to you to see for yourself and check your instructions.
If there is no notary wording on the form, and your instructions didn't say to inculde a loose acknowledgement, then you did nothing wrong by not notarizing.

As everyone has already mentioned, have them fax back the page. However, I am familiar with WAMU's policy and I find it strange that they want you to pay for shipping back. When I was with them they accepted a faxed page notarized by the NSA and the original FEDEX'd back. It can be considered a trailing doc for post closing to follow up on.
Two people I personally know never hit the NSA with the shipping charges. I guess their policy changed again for the 1000th time or you have someone overly doing their job.

I closed a loan the other night - not a WAMU, but there was some goofy form in there like you are speaking of. It was a Prudential Relocation loan from Chicago Title. ALL of the notarial wording was wrong. It named the venue as some location in Arizona. I had to change them all. Then, out pops this form that HAS the correct venue on it, and it states "sworn to and subscribed before me, the undersigned officer, blah, blah, blah..." and then there was NO PLACE for the notary to sign. Nada - zip. There were 2 places for the borrowers to sign, but nothing for me. Feeling left out, I attached a properly executed, blank ack and put a sticky on the original doc which read "So, where is the notary supposed to sign?" I know; I'm a smart alec. Can't help it sometimes. These types of stupid errors, to me, are inexcusable.
