power of attorney blank

one of my clients halted a signing when she came to a blank "tax and mortgage insurance" form in one of the loan packages from a mid-West mortgage company...she claimed that anything could be filled in and so declined to sign. Is this a common form? I am just starting to do more of these signings and hadn't seen one before in the loan docs.

Did you explain to the client that those were probably standard documents and had no significance unless they were filled in and notarized?

The ones I have came across were giving the company power to make any changes on typo's no changes on numbers though.
