Survey Affidavit
Can you please help a newer agent. I have a signing at 11am and have a survey affidavit that has no names preprinted and the notarial wording is not the same as the normal PA wording that I am used to.The wording is Executed and Sworn to this day of (blank space) signature? (blank space) Signature? Sworn to and subscribed before me, by (blank space) (blank space) of (blank space) on this day of .
I have never seen this wording, what would go in those blank spaces. Thank you VERY much for your help. This is my first time with this company and I would like to keep them.

Because you have an appointment already, here is the answer. You are NOT READY to be a Notary Signing Agent if you have to ask a question such as this!

Rather than trying to figure out what needs to be done to the preprinted certificate to make it a state compliant jurat, simply attach a PA compliant loose certificate.
Whenever attaching a loose certificate, it is best, imo, to make one diagonal line through the preprinted certificate, write "See attached certificate" on the notary signature line and complete a compliant loose certificate, annotating the loose certificate such that the certificate cannot be used with any other document.

this is PA compliant although it's got extra 'junk'. 'executed' and 'subscribed' are similar so that's not a problem. I personally ignore extra stuff like ... before me by ___ of ___ ...
Paul said to attach a loose cert but in PA it's so very basic if you don't like what you see just hand write what's correct (or purchase a stamp) on the page. Most times there is room.
PAN has said we should not draw a line through the incorrect wording and indicate 'see attached'. I personally feel that's dumb as can be - I think their reasoning was that we are changing a document and we have no right to do so - or some other such nonsense. Marlene are you reading this? What's the 'official' position?
