Does anyone have a form or wording for a minor to travel out of country without a parent?

It all starts with getting a lawyer. As a notary public it is considered "unlawful practice of law" (UPL) for you to supply these documents.

My daughter traveled last summer and the organization she went with supplied us with the necessary paperwork. You are going to need an attorney on this one.

Law Depot has fill in forms for this
I've used them. I'm not paying an attorney for something I can do myself.
I needed a consent form for a Horse Clinic a few years ago, and the 4 attorneys in my office couldn't figure out how to draft one correctly. I did it myself, and the organization now uses mine for all their clinics in the US.

if it is a personal ? , check with the airline. They are usually the one who asks for it.

Dorothy, I got both the one that Lisa gave and the one that I gave to work.
I shortened mine without all the gibberish at the end. Just type it out longhand if clicking on it won't work.

Our middle daughter went on a mission trip to Ecuador a few summers ago. The church had all the necessary paperwork. But, we also had our attorney draw up the legals and gave custody of her to our minister and our accountant (who went along) for the 4 weeks they were gone. This was for anything including medical treatment.
