Not sure if this answers your question, but I have seen a form titled "Indemnity and Affidavit as to Debts, Liens, and Possessions" included with some Countrywide loans. At the very top of the form, above the name of the form is written in bold letters "To be filled in personally by Seller or Borrower in His/Her own handwriting." There is a notarial certificate with Jurat wording at the end of the form. By filling out the form, the borrower is stating they have owned their home since (year), there are no undisclosed liens or taxes, and they are not currently making any major home
improvements.
