I would add a Loose Acknowledgment Certificate to the needed document making sure that your loose attachment references the attachment...(clear as mud?)
"Doc"

Not all affidavits require a jurat
If you mention them specifically, I might be able to shed some light. Many of my closings have CW or FS as the lender. It has been my experience that the docs they want notarized come with the preprinted notarial formats.

Yes, add the venue, it states right in our statutes it needs to be there. Just print nicely and you will be fine. I add venues to all docs that dont have them. Never a problem.

I have added State of (State), County of (County) if there is room. Otherwise, attach a loose certificate following your state law.

I have attached a loose vertificate when no pre printed.

Some have the venue printed at the top of the doc
usually at the top left hand corner before the statements, and not near the notarial certificate if it's a jurat. In FL, that is sufficient and doesn't need to be added to the cert, but can be w/o any problem.
