Certification of Trust

I am getting ready for signing tonight...BOs will be signing as trustees. No problem. But there is a totally blank Certication of Trust form, with only one signature line. And it's supposed to be notarized. Anyone ever run across one of these?

i don't believe that you.......

can notarize a blank or incomplete document......at least in wi and most notary assoc will advise against it because people may add things after the fact in the blank spaces......jmho

Borrower need fill the certification, ( coy it from his trust) then sign
