The person who is requesting the notarization is responsible for knowing what type of form to use. As a notary, you cannot make that decision.
If the client does not know which form is required, you should instruct him or her to contact the agency for which he or she will be submitting the notarized document (as to the correct form).
I also suggest, that you highly familiarize yourself with California's Notary Public Handbook. Many notarial questions are readily answered in this reference tool.

You can also show the person loose certificates. After reviewing them, he or she can then decide which one to use.
If the signer still does not know or is uncertain, again, he or she should contact the agency for which the document will be filed.

Pamela is correct.
It is Unauthorized Practice of Law (UPL) for us to tell people what forms they need, help them fill out the forms, or give other legal advice.
The tendency is to want to help.
