Request for Full Reconveyance

After the notarial wording of DOT at the bottom there is a paragraph "Request for Full Reconveyance" Under the signature line says (Trustee) without borrower's name. Does the borrower sign this? If yes does he need to print trustee after his signature? TIA

The Deed of Trust creates a lein. Reconveyence is the removal the lein. Signed "whenever".
