Stamping W/O Notarial Wording

Just wondering why there are so many docs out there that request a stamp on the document but don't have ANY notorial wording? I notarized a signature for a Request for U.S. Passport of a Minor today. This document apparently came from the State, did not have notarial wording but requested the document be stamped and had a spot for a notary seal. I attached an All Purpose Ack. I was trained that it is against the law to notarize anything if it doesn't have the correct verbaige, doesn't the State know this? Am I wrong on this?

You did exactly the right thing Smile You must know and follow your own state's laws. In CA the notary is not to apply his/her seal and signature without the accompanying appropriate notary wording (ack or jurat).

In Maryland we can witness signatures on documents withut notorial wording. There is a procedure in our notary handbook for those situations where we are presented a document without any wording.
