Name Affadivit for Non borrower

I just went to a signing where a non borrower had to sign a name affadavit, but their name wasnt on the form so I didnt think she had to sign. I didnt get it signed and now have to go back. Have you encountered the same? Ive been doing this a while and only remember til, deed, rtc and sometimes compliance agreement but usually the form would have their name on it.

I have had packages that include one for the Borrower and an extra blank form. I have the Non-Borrowing spouse complete the blank one.

it is sometimes a document that the lender wants executed by the non-borrowing spouse. And sometimes identification documents will need to be filled out for the non-borrowing spouse. Each lender is different in the docs they want executed outside of the normals when it comes to the non-borrowing spouse. And sometimes the Title Comp will have documents in there that will need to be signed by them also.
