Sue is actually correct. Seriously, if you don't know California law, you could do serious damage. I'm sorry, but doing a California Acknowledgment correctly is basic knowledge.

It is not in California.

I am sure you are going to say that Sue, Joan and I are "mean" BUT
You are a California notary and were required to take a class before obtaining your commission. The question you are asking is for basic notary stuff that you should have learned in your class had you been paying attention.
You also have an EXCELLENT California handbook which covers every aspect of notarial certificates.
A lot of states do not have the excellent resources California - and Florida - have. Be thankful you are in California.
Please study your handbook and you will find your answer.

Thank you for your respectable reply. This is not from California, but Minn. I have not practiced in California for four years. Yes, I can fill in the blanks, but exactly underneath it has the date. Just confusing to repeat it again.
Thank you.

OK - what do you think goes in the blanks?

Bearing in mind the Latin initials "ss" at the end of the line with the blanks

hint: start a new line after the comma and ignore the ss at the end. It should start to look familiar.
