Certificate of Residence

Who fills in the Certificate of Residence on the mortgage when it is asking to certify the address of the within- named Mortgagee and it is to be signed by the Agent of Mortgagee, the SS says I do it but I say they are wrong, tried to contact lender got no response yet

Mortgagee = lender, mortgagor = borrower n/m
