Help!

I am struggling with what to put in the blanks on this notary acknowledgement. I have closed a bunch of loans, but this is the first time this acknowledgement has come up for me. This is a CitiMortgage loan doc and I am honestly lost . Thanks for the help.

I,_____Colo Notary___ a Notary Public in and for said County, in said State, hereby certify that signed to the foregoing instrument, and who _________________ (would this be IS?) known to me, acknowledged before me on this day that, being informed on the contents of this instrument, ________________ (would this be he or she?) voluntarily on the day the same bears the date.

Who is the borrower's name; the other is he/she/they, what ever applies.

I'm with Misty. Why not just attach a loose ack that complies with your State's requirements. That's an odd ack for sure.
