All-State Signature-Amy Smith-Mills, KY

Took a last minute signing from her in August, a SC search came up empty so I took the job. Now she doesn't return calls and fax has been disconnected. Does anyone know anything about this company? If anyone can provide a good address please let me know. Phone calls go unanswered. Reverse look-up on anywho.com doesn't help.
