Add-on to previous post regarding CRV

Who actually is to fill this out? Last week I had a signing for a purchase (rental property) and included in the package was a blank CRV as usual for purchases (Certificate of Real Estate Value). I just had the buyer fill it out the best he could and sign since it did have to be notarized. It was only the borrower and I at the signing. I am just asking because of my last post where my NSA friend was asked to fill it out and I never do. What do you do with it if it is only you and buyer at signing?

This looks to be something specific to Minnesota. From what I can see of various sample forms it is to be filled out and signed by the "buyer, seller, or agent."

If memory serves, this is a form I've seen with some short sales - and perhaps other seller's packages. It's something that looks like it should have been filled out in advance, but I've yet to see that happen. We got the information from the RE agent and the seller filled it in. It's not something I would have done for the borrower, even if it wasn't notarized. (I don't remember, off the top of my head. I haven't seen one in a while.)
