Name Affidavit
Hi. This may be a dumb question but need to be sure before sending back docs. The name affidavit form where the notary signs. Sub. & sworn (affirmed) before me________________, on that line is it the notary name or customer name?

Notary's name.
In the notary/signing agent business, there is no such thing as a dumb question. Details and accuracy are key components when notarizing. Therefore, always feel free to ask questions.

"...before me (notary name) on (present date) by (name of signer) who produced (ID) as identification."

Well.... i don't want to say that's a dumb question. It's a good question.... but one that somebody learning to be a notary should ask. If you're already a notary, you should know this. It's kind of like a taxi cab driver asking, "Where do I put the keys?" But that's okay... we all have moments like that. May the car is one of the new hybrids or a car where you push a button?
What goes in that line depends on the rest of the wording following that line.
From what I can see right there... it would be your name....
".... before me LUCY JONES, Notary Public, ...."
But it might not... what is the rest of the wording?

Another thing...
I don't know about Illinois....
But you said "before sending back docs" -- does that mean they're already signed?
Why didn't you complete the notarization at the table?
I know that some notaries don't do that as a matter of time conservation, but I find it opens yourself up to mistakes and possible (at least in CA) legal issues. That's why I try to make sure the certs are filled out (just not signed/stamped) before I get to the appt, if I can.

The signing is this afternoon. Just looking things over before apt. Sometimes wording just doesn't always make since so it's nice to just clarify myself with others. Thank you.

You're right Mirian. The wording following the blank space is important. I seen affidavits that say something like "... _______________, known to me or satisfactorily proven to be ...." In this case, the name(s) of the signer(s) would be in the blank space.
