Verification Certificates in other states? just curious

Does any other state have a similar certificate or a notary form which is called a verification?

I was doing something for my HOA and was reminded of this type of notary cert…became curious about it because it’s almost never mentioned among posters in this forum.

I don't see any difference between the "Verification" and a "jurat" other than the purpose and process are explicitly stated in the Verification. That is to say, when a notary signs a jurat that states "sworn to and subscribed", that means that an oath or affirmation was administered ("sworn to"Wink and that the affiant signed ("subscribed"Wink the document stating that the facts are true and correct.

Am I missing something?

I don't think so. I am just curious as to whether or not there is such a certificate in other states.


Our Texas notary definition say this:

VERIFICATION: A formal declaration by which one swears to or affirms the truth of the statements in a document. Also, the statement of a Notary Public that the person appearing before the notary has been properly identified as being the person purported to be appearing.

Ah! Does TX require the affiant to be identified when a jurat is used? I remember reading that some states do not require the notary to absolutely identify the signer, only administer the oath and watch the affiant sign the document. If that's the case, then a VERIFICATION is a jurat with identity verification.

In Florida, no matter what form is used, ack or jurat, the signer must be identified (and the form of identification must be noted).

Brenda,

I thought the same thing as PAW when I read this...They sure seem to be the same thing.

In the Illinois State Handbook, page 9:

"Sometimes referred to as a "jurat," verification upon oath or affirmation is a declaration that a statement is true and was made by a person upon oath or affirmation."

Our sample "jurat" forms even say "verification upon oath or affirmation" at the top and they use the typical jurat wording:

"Subscribed and sworn (or affirmed) before me on________________by__________________.

We do not have a certificate that I know of with the other wording you mentioned, just the jurat.

Could be used in OR - our handbook gives a

sample oath:

"Do you solemnly swear (or affirm) that the statements in this document are true (so help you God)?"

The sample cert simply (Verification Upon Oath or Affirmation) says "Signed and sworn (or affirmed) before me on ____, 2009 by _____________." (add usual jdx and sig line.)

Your example cert would work in OR after taking the oath.

PAs form says

I, Sam Signer, swear or affirm and verify that the statements made in the foregoing are true and accurate to the best of my knowledge, information and belief.

Sam signs and then follows a jurat with the notarization.

When I worked, the past perhaps 15 years, we used unsworn verifications on complaints filed with the court.
