Proper Notarial Wording...
That's all you need. Ask the signer if they want and Acknowldgement or a Jurat.... not your call. You'd likely just attach a loose form.
NOW... it may be that they will need to get a certification of your signature.... the Ukraine is not a Hauge Convention country, so an apostille won't work. It's NOT your job to get that certification, though... and it's a long, tedious process. It's also not your job to determine if they need certification or not.
All you have to do is notarize the signature as usual, using a loose form if you have to.

Re: Proper Notarial Wording...
Thank you. I have one more question. I had to notarize 17 pages. Do I charge them for each signature? $170.00 plus travel fee?

That's up to you...
I suggest reviewing your handbook and figuring out your costs for this by travel and time. Are they all the same person, or 17 different people?
The MAXIMUM you can charge is $10 per signature. You can charge less if you choose. This is a common mistake CA notaries make... they think that CA law dictates $10 a signature --- NOT TRUE. If I want to always charge people $5 a signature, I could.
In fact, if you're dealing with 17 signatures, it may be wise to drop your fee a bit... or not charge a travel fee at all if you think they're shopping around.
I know that if someone called me to notarize 17 signatures, and if the travel were reasonable enough... I'd possibly say, "That will be $10 per signature. Usually, to go out to XXXX, I charge a $35 travel fee, too, but I'll waive that this time."
Just remember that 17 signatures can take some time and you have to journal them all.

Let me add to that!!
You said "17 pages" --- the number of pages doesn't matter. What matters are the number of signatures that you are asked to notarize. It could be one page with 17 signatures, or 17 pages each with their own signature... or some other combination.
Are you sure that they want 17 signatures notarized? Or is it a 17 page document with one signature? That's a big difference.
