If you are a notary you should know the answer, if not shame on you. FYI in that space it goes the person you are signing.

Oh dear. You are witnessing the signature of a person. 'By'
would most likely be the signatory.
'Borrower' would be the borrower.
'Day' would be today's date, 'of' would be the current month, and again, 'by' would be the signatory.
'Notary' would be you, the Notary Public, and you would sign that line. Then you would stamp your seal.
Is this your first encounter with performing a notarial act?

It would be the name of the person who signed the identity affidavit. The samples for a Jurat in the Colorado handbook do not have the words "by__________________, proved to me, etc..." but it does state that these words can be used to clarify which signature the notary is certifying. Just remember to give them the oath before they sign the document.
