NOT ROT notary bond question

I took my bond to the notary. Neither of us could figure out what to put on the very last line of the jurat/oath of office. It looks like this: ______________________, _________ .
We figured it could be the date or the city and state. However the date is already stated.
Does anybody have any ideas?
