Washington notaries: need jurat form
A friend is allowing her underage daughter to go to Canada with her aunt & uncle and has written two letters allowing this couple to take the child across the border (one letter for permission and one letter for permission to begin emergency medical attention if necessary).
I know that I need to have an affidafit or jurat (their choice) accompany those letters, but currently I am out of jurat forms (rats!). They would like me to meet them tomorrow AM to have this done.
Can anyone forward me a jurat form that I can print and take along? Many thanks!

Patti You can just type one up following the guidelines in your state handbook.

I have a handbook from the SOS of Washington. It's called "The Laws Relating to Notaries Public" 42/44 RCW and 308-30WAC. I called the SOS office and requested. It's come in handy a few times. Good luck.
