Here's a first

This form from the Florida Department of Business & Professional Regulation [http://www.myfloridalicense.com/dbpr/abt/forms/documents/abt6008AppPackforImporterorBrokerSalesAgent.pdf] has a notary certificate on page 6 - where the NOTARY is supposed to check off whether the document was sworn to or acknowledged. The document obviously requires an oath but I have NEVER seen a document have checkboxes for the notary to select the type of act being performed.
