How would you fill this out?

Any help appreciated - lots of fill in the blanks and want to confirm my thoughts are correct. Thanks!!

Okay, just about how I thought, and I REALLY appreciate it. All the blanks had me stumped so the last one (with the VP part) totally helped me out.

One more detail I left out...
If the person is signing as an individual, and not as a representative of an entity, you could just cross out "as the blank of blank, etc". However, in that case, they should have used an individual ack in the first place, instead of a representative ack.

Shauna, by the way
I have been catching up on old posts tonight, and I read your post from a couple of nights ago regarding your first signing. It looks like everything went fine, but I wanted to say "Congratulations" and I'm sure you'll do great, for no other reason than that you were so concerned about making sure you had done all the preparations correctly. Then you came back and talked about the minor issues you had at the signing, and it sounded like you had them completely under control. I just wanted to agree that you're going to do great if you continue to be that conscientious about your work!!! Good luck!

Aw, thanks so much! My goal is just to do right by this profession. So I figure if I'm a little overally neurotic it's probably good.
Thanks so much with help with figuring out the above. Got it done and was in and out in less than five minutes. Luckily it was the Title Co's fault for not including it in the loan package and not mine (WHEW!!).
I also owe a HUGE thanks to this board. Being able to read it while getting myself ready to hang out my shingle was a gigantic help.
