names in acknowledgement section question

When acknowledging a signature where the signer has signed both former name and current name on a deed, do you write in both names that they signed as in the notarial certificate? I know I read about that on NotRot but can not bring it up in the search. This must be my day for questions........

In Florida:

"The foregoing instrument was acknowledged before me this 30th day of November, 2010, by Mary Jane Smith, who represented to me that she was formerly known as Mary Jane Doe and who produced a Florida driver license in the name of Mary Jane Smith as identification."
