Attention to detail - LACKING

Over the last 3 weeks I have had 3 loans (2 USAA, 1 BofA) that have had either titling issues on the deed or borrower name spelled wrong...

What is going on? (somewhat rhetoric)

Not sure...but I did a USAA recently that was an

absolute mess!! Docs out of order (e.g. TIL and definitions separated by numerous pages); many duplicates; bad format on forms of text to signature lines. And yes - they came directly from USAA - no stop or filtering along the way...
