When do you need an extra print fee?

I have established clients with packages 350/380 pages(both sets)that I don't charge additional because they send me 10-15 deals a month and in those 10-15 loans there are a few with 80-90 pages per set so at the end it evens out.

But what about those clients that calls you once every 3 or 6 months and than it sends you 15 attachtments with 185 pages per copy. How do you guy/gals handle the extra print?
