The client needs to provide the document. (or is it for yourself???)

Rochelle Not sure what you are really asking. Your original message was about a document for parents when they go on vacation out of the country and leave their childern here.
What document would be copied???? You need to be more specific on what you need to know.

Why would you need any docs to go on vacation and leave your kids? Leaving them alone? not a good idea. Minors being left with someone? Why not a limited POA (Medical directive?). You could always call social services and ask, I'm sure that will go over big time.

I've notarized a document for a child that one parent and/or grandparent was taking on a cruise for a friend when he and his wife were separated. It might be that type of document (for the child to go out of country).
In any case, I wouldn't be the one providing any docs for anyone in that situation. They would have to present the docs to me.

Most airlines will accept a notarized form or letter signed by the non-accompanying parent(s) that contains, name(s) of non-accompanying parent(s), name(s) and date(s) of traveling child(ren), name(s) of adult(s) whose custody child(ren) are put under while travelng, destination, departure date and return date. This should also include a limited POA, medical directive.

The document drawn up by the parents should state whatever the situation is. You cannot write it for them. The parents can choose if they want an ack or a jurat.

Rochelle--Are you a parent or family friend trying to get information for these folks or did they contact you simply because you are a notary? It seems to me you might be getting into the murkey waters of UPL--a definite no-no! As notaries, we only witness & acknowledge signatures. Giving legal advise is strictly prohibited. As a Notary, I would only advise them on where they might call to get the proper information. let them take it from there and then, if you wish, you can notarize the document THEY provide.

Re: Vacation Document - Simple, Simon
Whomever is leaving their kids needs to write a statement that basically states what they wan, and the time period the statement is in effect.
Example: "John Smith & Mary Smith are parents of Cindy Smith and James Smith. residing at (address). We grant permission for grandparents Gary and Kathy Smith to provide any necessary medical care for our children during the period May 17- May 31, 2011." Or something along those lines, depending upon what they want done.
But THEY have to write the statement with their wishes, and you have to ask them if they want ack or jurat wording; then you can notarize their signatures on the statement, assuming all the proper notarization elements have been completed.

Yes, and I don't understand why you find this so bizarre
Parents go on vacation all the time and leave the kids with grandma, an aunt/uncle, etc.

Are you saying my understanding is incorrect? My post says

No, what you said was

...as if it was unusual or a crime to leave your children when you go on vacation.

Re: No, what you said was
Ahhh Okay got you.... As far as the crime thing, here in Calif because of all the "child abandonment issues" with people leaving kids alone to go to the neighborhood "whatever" It is not a good idea. If Social Services or a teacher, neighbor were to to report such the children would be taken into custody by child protective services. If the children are left with family members and have something such as Roger_Oh mentioned that would be O K in my opinion. There's really not enough information (as Sylvia stated earlier) to know exactly what's being requested. Thanks.
Also I guess I might be weird because while my children (5) were living with us, they always went on vacation with us because we vacation one month each year. I would not put them on anyone, besides it was a 'family vacation.

In FL it is a crime if the child is under a certain age. Many other states may have the same type of law as well.

Re: No, what you said was
I don't doubt that it is a crime to leave children ALONE. But the OP did not state those were the circumstances, and I highly doubt that the parents intended to leave the children alone.

We do, in fact my wife and my daughter living at home are both teachers and required by law to report such.

Yes, mandatory reporters. I think where kids are concerned,
we're all mandatory reporters...

A multitude of reasons, imho. No info on ages, etc. Also,
the OP did not say the kids would be ~alone.~
One last thught - there are family vacations, where everyone gets away for some fun; vacations for the kids away from parents,(grandma's, away-camp, etc.), and vacations for parents away from the kids (Paris in the Springtime...)
Not being mean, I just mean there are lots of plausible, sensible scenarios.

That's what I mean, Susan.
I doubt that we are talking about parents leaving their 4 year old triplets at home ALONE while they fly to Bermuda. I would be willing to bet that the children are either going to be left in the care of some relative/other adult, or it's a few 17 year olds that may be capable of taking care of themselves with some other relative available to assist with any emergencies.
