  Help!
  I was approached by a customer who wanted a document notarized that had all the info on the first page but on the second page was blank.  The second page consisted of the seller's signature and buyers signature and then they asked for the notary's signature with the notary's commission exp date.  That's it.   There was not any correct notary verbage on there.  I told them that I could not notarize it.  My question is... can I still have them sign it and attachment my acknowledgement form to it?

  You need to show them the certs and let them choose whether they want an ack or a jurat. You cannot choose for them.

  Sorry... sending this again.  Not sure if you got the last message.  Just double checking.  Once they pick the form, its still ok to notarize the doc even though the second page is only their signature? Nothing else.

  Yes attach a loose ack or jurat depending upon what they choose. Also, I would note on that sig page that a loose CA (ack or jurat) is attached. The certificate should be stapled to the other two pages. Also, it's good to use a loose cert that has a place for identifying information on the bottom.

  To expand on what Shoshana said about identifying info, I would include the fact that the certificate is attached to a two page document, plus a file or escrow number, document date, etc.
