notary identification statement

A title company has requested that a form that includes the borrowers address, name and ID be notarized. It is not signed by the borrower.
I have not seen this before.
Any comments?

I get them all the time. Just sign it. And you can either call them or stick a note on it that there is nothing to notarize. Only once did I have a problem with a Title Company and we had to take a different approach (use a different form).

I write on the document itself that I cannot place my seal on it. I will sign them.

And don't fall for their trick when a line beside your signature says to "place notary seal here to prove commission", or something like that.

I can't do that today in my state as a notary, but maybe in 2013.

In Vermont, in addition to the usual powers, notaries may "perform any other act permitted by law." (24 VSA § 445) A law that has been passed in the Vermont House and is being considered by the Senate will require requests for vital records sent by mail to include copies of acceptable ID (driver's license, passport, etc.) AND "shall include a notary public statement of identification of the applicant." (Senate calendar for April 5, http://leg.state.vt.us/docs/2012/calendar/sc120405.pdf ) I suppose former Vermonters who have moved out of state and need a certified copy of a vital record are out of luck, cuz all those out-of-state notaries can't do notary public statements of identification.
