Forgot to sign the last page of the DOT (CA)

DOT, all 15 pages has her initials, but I forgot to get the signature on the last page. I did fill out a notarial certificate. She has to sign again. I guess I would have her sign with the date when we meet and add a new journal entry and attach a new certificate. This is the first time this happened to me.

How many times to you guys recheck the paper work before sending it out. 3 times for me. What is your opinion on this subject. You work long hours, you get tired, it is easy to miss a signature or an initial.
