Signing Agent or Notary E&O Insurance claims

Does anyone have first hand (not necessarily you) knowledge of claims, particularly litigation, against a notary/signing agent for a claim arising from a signing, or from a notarial act? It would be interesting and instructive to follow the course of the litigation if you have a citation to the case, names of either party, and court or jurisdiction in which it was filed.
