I need some help with notary wording
Before me, john bouchard, on this day appeared, Borrower, known to me (or proved to mo on the oath of ______________________ or through ______________________ to be the person.....
What wording should go in the two blank lines above.

first blank: credible witness's name--if allowed in your state second blank: State Drivers License

Attach a loose cert? You then have the wording you like

It looks like you are supposed to use "substantially" the certificates shown at
http://www.mass.gov/governor/docs/eo455finalrevised.doc
If you decide the certificate on your doc is substantially the same as the one's Mitt Romney wants to use, I would complete it differently depending on how you IDed the signer.
1. You know the borrower personally: ...Borrower, known to me (strike out ) to be the person...
2. Your friend Fred Friendly swore to the identity of Borrower: ...Borrower, (strike out) proved to me on the oath of Fred Friendly to be the person...
3. You used a driver's license: ...Borrower, (strike out) proved to me (strike out) through a Massachusetts driver's license to be the person...

I've found the easiest quickest way to deal with that form of acknowledgement is just to underline "proved to me" and then, in the next line underline "through" and insert "TX DL" in the blank. No strikeovers. Never had one chalenged.

If you are a Notary, you should know, without question, to use VT_Syrup's suggestions to use the 'Romney' wording as referenced. That is an absolute.
