Five Wishes Document ...
... Is anybody here familiar with a document titled 'Five Wishes" ? And have you been asked to notarize the signers signature on it? The document itself has parts that are similar to the Ca. Advanced Health Care Directive but says only that it needs to be witnessed, & notarized only in a few states. The normal Ca Advanced Health Care Directive gives the signer a choice of either 2 witnesses or having it notarized. I have a client who wants it notarized rather than having his signature witnessed, & I am thinking that might cause the document to be rejected by his health care team. (The client thinks that notarizing might make it more official). Can anyone shed some light on this document?

I've not heard of this particular document but I do notarize a lot at hospitals and rehab centers. After reading this form, it appears to be (IMHO) an easy to understand, simply worded Advanced Health Care Directive.
According to the directions on this form, for California, it only needs two witnesses. I'd let the client know that it's best to follow the directions on the form. I'd offer to be one of the two witnesses - signing as only a witness and not a Notary public. In MY experience, when I'm one of the witnesses, in the client's mind, it's the next best thing to being notarized. JMHO
