My computer crashed a week ago and I lost everything. Could someone please e mail a blank Servicelink monthly billing statement to me. I had the form saved in word and it is gone!

You need to consult with Scriba/NM on the absolute minimum requirements for anyone who presumes to practice signing agency. He will not mince words in letting you know that he sets the standard.

OMG Pat, I am so sorry to hear that your computer crashed...
What a terrible thing to happen. I wouldn't wish that on anyone. My heart goes out to you.
But what I do wish is that we could get Service Link outta business with their lowballing fees. (no offense!) I've been reading on other forums that people are really having a hard time getting paid by them. Not good things at all.
They called me yesterday asking me to join their data base. Of course I told them to send the application but ain't no way in HELL I'm keeping them in business.
I just realized they didn't send it so I suppose they got the hint when I told them their fees are way too low for me.
Maybe they're reaching out to other notaries because the one's they're not paying stopped working for free? Oh YAAAAY, I say! 
Good luck with your computer. I really feel for you.

First you go off about taking low paying jobs from a company you like (I do too, for that matter.) Then you go off about SL who you have never worked for. Know nothing about and as usual run your mouth.
I have worked for SL for 9 years. I have ALWAYS gotten paid. They pay every month like clockwork, period. Yes, there are the idiots that will take $45 for a signing. But, if you don't accept it, they will pay you your fee. I make between $100 and $250 per signing, depending on how far I drive and I do a lot of jobs for them each month. They pay for no signs (usually always my full fee.) If you bill them for the signings you do each month, within 2 weeks tops, you have your check.
On occasion, if you don't get paid for a particular signing, it's because the closer, did not close out the file correctly. If you give them a call, they will and then you will get paid.
You want to wish company's out of business. Wish the ones that deserve it. But, know what your talking about first.

as she has been loyal to me. Furthermore, I will say again that I was referring to the one star slamming's that were posted, just in case you didn't read above.
What I think is hysterical though is your comment "...as usual run your mouth". I think that was rather harsh (as funny as I found it-it still makes me laugh), but I guess you're entitled to your very humble opinion. I can appreciate your kind words of wisdom and respect you for telling it like's you sees it. Oops, there I go, running my mouth again. I'll try and curb my sharp tongue. 
Anyway, I do know what I'm talking about because I have been called and offered their $50-65 closings. Not to mention, thank G*D, I don't have to speak from the vantage point of not getting paid; I'm grateful to say, you are right, I don't have that to fall back on.
I am happy that you shared your experience in that their long time notaries continue to get paid their due worth. But I would wonder what's going on since they are reaching out to widen their data base. I hope that the company continues to use you and give you your full due and worth. If you look on other forums, there are people having big time issues with getting paid. Maybe it IS the notaries fault, maybe it's not. A number of them are attesting that they HAVE called, and emailed, and called again, to no avail.
I understand why you would be so upset. I would be too if it was a good company that I worked with and someone was wishing them out of business (after all, that's exactly what I did!! not that anyone was wishing them out of business, but...) I apologize if I affected you in any way. I'm sorry you feel that way and hope you feel better for getting it out of your system.

You completely missed my point. Which was you doing what you constantly keep criticizing, chastising, verbally abusing, and going on tirades about others for doing. Well that is when your not forgetting your on a professional forum and typing like a 12 year school girl, tee hee. Or, going on about a company you really know nothing about. But, of course that's just my ever so humble opinion.

You have to realize that if you're rural and you're one of a handful of notaries in your travel coverage area, you most likely will be paid timely. Not by choice, but by necessity. A SL will need you again, so of course they won't stiff you. But if you're a notary in a busy metropolitan area, you're one person in a SEA of notaries from which SL can pick from and then stiff. So based on your locale, your experience CAN be that you always get paid. But other's experience CAN be that they got lowballed or worse (stiffed).

Just an FYI there are a ton of signing agents in my area. The problem with payments from SL some people experience are because they don't bill properly.

Perhaps it depends on division? SL meets my fees
I'm no lowballer, and ServiceLink meets my fees ($XXX) most of the time. (The rest of the time they can find somebody else).
My volume with them has gone down a lot in the past 12 months but I still do 1-2 closings a week for them at very competitive fees and I don't have payment problems with them.
By the way Pat, I sent you the forms.

That's why I use Carbonite online backup system
It's very cheap and then if you computer crashes you can get everything back quite easily.
