Chase Notarial Evidence form
I just read trhu a bunch of forum comments on this, and I may be blind, but I did not see if anyone had used it , and just listed the Notarized Chase docs, and not LSI's Notarized docs, and found that to be OK. I have my first job with the new form tomorrow. TIA, J.

I comnplete them regularly. They are included in amost all Chase documents and are all the same.

What I need to know is if you list the LSI Notarized forms such as the Borrower's Title Aff and the other one, as well as the Chase DOT, Signature, and ID affs .Thanks.

I don't.
I just list the lender's notarized docs. No repercussions as of yet.

Same as Donna - I have only listed the Chase docs on mine
I made a master copy with notary info and the notarized Chase docs filled in, and all I do is print a new one and add borrower's info and loan info. I have two master copies - one for single borrower and one for two borrowers.

I have had to fill this out several times already and it does take a few mins no big deal..I fill in all documents that I notarize..Not just the lender ones..

I disagree, this is a lender form. I don't understand why anyone would want to to create more work for themselves.
