I was looking it over and don't know what to put in the blank lines

I am confused can someone help?

This looks like it might be an acknowledgement. Is it? If so, you might want to attach a loose cert with your state's notarial verbiage.

Thanks Shoshana. There is an Acknowledgement just before it. That is why I am confused. There are two wordings to be notarized.
