maybe this will help you from a past forum http://www.notaryrotary.com/archive/forum/2004/October/Medallion_notary_.html
I have never been asked for this, and it seems must come from a bank or financial institution when I googled it.

This has nothing to do with us. It's a financial & banking
thing that has to do with securities.

It's a bank medallion, which signifies that the bank is certifying the signature of a depositor. I just recently had to get one on some paperwork regarding my mom's estate. Had never heard of it before either, but the guy at my bank said "no problem" - checked my ID, went into a back room, and came back with a stamped document. The colors were different from what you said, but it was definitely multi-colored.

I had never heard of it before either UNTIL a few weeks ago when I went to notarize a document for a lady who showed me the paperwork regarding the "medallion". I told her it looked to me like it was up to the financial institution that she was dealing with to take care of that - and, after she made a phone call, I was right! YEA.
